Position Summary

The Primary Care Research Institute (PCRI) in the Department of Family Medicine seeks a Case Manager responsible for case management and community outreach with criminal justice-involved or formerly criminal justice-involved individuals with co-occurring substance use and mental health problems in the Erie County Comprehensive Opioid, Stimulant, and Substance Use Program, using the MISSION-CJ (Maintaining Independence and Sobriety through Systems Integration, Outreach, and Networking – Criminal Justice Edition) evidence-based model.

Duties and Responsibilities Include:
  • Meet with participants to discuss their needs in the community (e.g., connecting them with mental health and substance use services, recreational opportunities, self-help groups, transportation resources, etc.).
  • Provide vocational/educational support to help participants maintain employment and find new employment/educational opportunities.
  • Collaborate, care plan and work with criminal justice entities (e.g., probation, parole, courts) to support participants in reducing recidivism over time.
  • Provide person-centered care planning and goal setting using the motivational interviewing approach.
  • Identify, educate and support caregiver network.
  • Introduce self-management tools and community support services.
  • Document care plan, client interactions, and linkages in Client Management System in a timely fashion.
  • Meet with community partners to assure grant deliverables are met.
  • Assist Project Coordinator with data management, grant reporting, and overall project management.

Minimum Qualifications

  • Bachelor’s degree in Psychology, Social Work or related field, in conjunction with two years of relevant experience. A combination of education and experience will be considered in lieu of the degree requirement.

Preferred Qualifications

  • CASAC, experienced with opioid addiction.
  • Familiarity with trauma-informed care.
  • Strong case management experience.
  • Peer Certified in New York State.

Physical Demands

  • Incumbent will be required to provide own transportation or otherwise demonstrate their capacity to meet the transportation needs of the job to and from research sites.

Salary Range

$45,000 - $48,000

The salary range reflects our good faith and reasonable estimate of the possible compensation at the time of posting, the role and associated responsibilities, and the experience, education, and training of the selected candidate.
